- Coverage request details: ZIP code, general protection goal, desired timing, age range, tobacco or nicotine use, desired coverage range, and general policy preference.
- Contact details: name, telephone number, email address, preferred contact method, and preferred time.
- Optional notes: the high-level context you choose to provide, subject to the warning above.
- Attribution and technical data: landing page, referrer, UTM values, gclid, fbclid, IP address, browser or device information, timestamps, page requests, and security logs.
- Consent and transaction records: request ID, submission time, the disclosure and version shown at submission, and your affirmative agreement.
Age range and tobacco-use answers may be considered sensitive or health-related information under some laws. We collect only the high-level answers shown in the form for matching; this website is not an insurance application or a channel for medical records.

At your direction, we may disclose your request and contact details to one participating licensed insurance professional so that professional can evaluate general fit and decide whether to contact you. The professional and any insurer are independent businesses and handle information under their own privacy notices and applicable insurance laws.
We may also disclose information to providers of hosting, security, communications, analytics, campaign measurement, and lead delivery; to professional advisers; when required by law or needed to protect rights and safety; or in connection with a merger, financing, sale, or transfer of the service.
6. Compensated introductions
A participating professional may pay Coverstead for an introduction. Depending on the law where you live, a compensated lead disclosure may be considered a "sale" of personal data even though you requested it. We do not disclose a request to unrelated data brokers or a list of professionals. If you do not want the requested disclosure, do not submit. After submission, you may ask us to stop later disclosures, but we may not be able to retrieve information already delivered to a professional.
7. Insurance applications and separate notices
Submitting this form is not an insurance application. If you later apply, a licensed professional or insurer may request additional identity, financial, lifestyle, and health information under its own forms and privacy notices. This policy does not govern that separate application or underwriting process.
